As a part of a new initiative of Aim Dental Group, Aim 4 Green, we are working on ways to become more environmentally conscious. In the coming months we are giving patients the option to fill in their medical history forms electronically. This new efficient way of updating medical history provides a secure way to update your information through a passcode protected link directly from our internal practice management software (Dental4Windows).

We will always have paper forms available to patients who find it difficult to use an iPad or would prefer to write their medical information on paper and hand it directly to front desk.

This is completely separate from the “My Health Record’ the government is introducing. The information provided to Dentists does not appear online.
